Once coded, the billing office (or a third-party vendor) generates invoices. Each charge is priced based on the hospital’s contracts with insurance companies—those contracts dictate the reimbursement dollar amounts, line by line.

Invoice → Claim Submission → Adjudication

The invoice becomes a claim. Hospitals file it, and insurers run it through adjudication. This is rarely a one-and-done process—insurers reject, reduce, and re-process claims multiple times before issuing a final remittance.

We bypass the typical coding debates and focus solely on comparing final payments against the actual terms of the contract. This is not a subjective review; it’s a contract compliance audit.

Industry averages suggest underpayments of 0.5% to 4.5%. Our audits consistently reveal much deeper gaps—often 14% or more—with averages between 18% and 22%. For a hospital with thin margins, this is the difference between solvency and financial distress.
